Abstract
국 문 요 약 배경 및 목적: 전 세계적 관심이 높은 암은 1983년부터 한국인의 가장 큰 시망원인이다. 방사선치료를 받는 암환자들은 보완대체의학 이용률이 높음에도 불구하고 이에 대한 연구가 부족하다. 이 단면연구의 목적은 이들의 보완대체의학 이용 수준과 이에 미치는 요인을 평가하기 위함이다. 연구 방법: 서울 소재 일개 대학병원 방사선종양학과 외래에서 치료를 받고 있는 암 환자 290명을 대상으로 설문조사가 수행되었다. 연구 방법은 인구사회학적 및 건강관리 특성, 암 환자의 인식과 치료 경험, 보완대체의학 이용 경험 등 3개 영역, 총 32문항으로 구성되었다. 수집된 자료는 SPSS Program 26.0 Version을 이용하여 빈도분석, 카이제곱검정, 로지스틱 회귀분석을 시행하였다. 연구 결과: 방사선치료를 받고 있는 암 환자의 보완대체의학 이용률은 63.1%였다. 가장 많이 이용한 보완대체의학은‘비타민․영양제(65.5%)’였으며, 가족․친척(54.6%)가 주 정보원이었다. 이용예측요인은 규칙적인 걷기(OR: 2.943, 95% CI: 1.434~6.043)와 암유형[유방암(OR: 11.463, 95% CI: 1.348~97.458); 위장관계암(OR: 2.027, 95% CI: 0.937~4.386)], 방사선치료 중 경험한 부작용 증증도(OR: 2.048, 95% CI: 1.105~3.796), 암 치료과정 중 환자의견 반영 정도(OR: 2.191, 95% CI: 1.171~4.099), 코로나19가 치료에 미치는 영향 정도(OR: 2.028, 95% CI: 0.988~4.165)였다. 결론 및 제언: 방사선치료를 받는 암 환자들은 보완대체의학을 암 치료 중 심리적 안정과 면역력 증진을 위해 사용하고 있었다. 코로나19가 암 치료에 미치는 부정적인 인식이 높은 환자가 보완대체의학을 더 많이 이용하였다. 많은 환자들이 의료진과 상의 없이 심리적인 안정을 위해 이용하고 있었다. 이러한 연구결과는 암환자들의 보완대체의학이용이 암 치료의 효능과 안전성에 영향을 미치는 지에 대한 추가 연구의 필요성을 시사한다. ◆ 주제어: 보완대체의학, 방사선치료, 코로나19 ◆ Keyword: Complementary and Alternative Medicine, Radiation Therapy, COVID-19|ABSTRACT Use of Complementary and Alternative Medicine among Cancer Patients Receiving Radiation Therapy during the COVID-19 Pandemic in Korea Youn Seon Lee Department of Medical Administration Graduate School of Public Policy Hanyang University Directed by professor Dong Woon Han Background: Globally, cancer incidence rate continued to increase over several decades, and it has been a leading cause of death in Korea. Despite the high use of complementary and alternative medicine (CAM) among cancer patients undergoing radiation therapy, limited information is available on patients’ CAM use during the COVID-19 pandemic. Therefore, this study aims to examine the patterns and characteristics of CAM use among patients receiving radiation therapy and identify the factors associated with the use of CAM among cancer patients. Methods: A cross-sectional study was conducted among 290 cancer patients receiving radiation therapy visiting an outpatient department of the Radiation Oncology unit at a university hospital in Seoul, Korea. The survey questionnaire included 32 questions on the use of CAM, sociodemographic characteristics, health-related characteristics, cancer patients’ experience and perceptions of conventional cancer care, and perceptions of the COVID-19 pandemic. Descriptive statistics, Chi-square test, and logistic regression analysis were performed for data analysis using SPSS ver. 26.0. Results: The prevalence of CAM among cancer patients undergoing radiation treatment was 63.1%. The use of vitamins and nutritional supplements was the most common (65.5%), and most patients obtained information on CAM from family and relatives (54.6%). Potential predictors of CAM use included the practice of regular walking (OR: 2.943; 95% CI: 1.434–6.043), breast cancer (OR: 11.463; 95% CI: 1.348–97.458) or gastrointestinal cancer diagnosis (OR: 2.027; 95% CI: 0.937–4.386), the experience of severe side effects during radiation therapy (OR: 2.048; 95% CI: 1.105–3.796), participation in the treatment decision-making process (OR: 2.191; 95% CI: 1.171–4.099), and having negative perceptions of COVID-19 on their cancer treatment (OR: 2.028; 95% CI: 0.98–4.95). Conclusions: Patients who perceived the COVID-19 pandemic negatively influenced their cancer treatment were more likely to use CAM, and many patients used CAM to relieve psychological stress without disclosing their use to healthcare providers. Our findings suggest the high patient demand for integrative cancer care, and in order to ensure the safe use of CAM, further study is warranted to examine whether cancer patients' use of complementary and alternative medicine affects the efficacy and safety of cancer treatment. ◆ Keywords: Complementary and Alternative Medicine, Radiation Therapy, Cancer, COVID-19, Cross-sectional study
