가토 두개골에서 Bio-glass와 자가골 이식에 관한 실험적 연구

Abstract

목적 : Bio-glass를 가토의 두개골에 이식하여 조직계측학적 방법을 이용하여 신생골 형성 및 이식재의 흡수를, 면역조직화학적 방법을 이용하여 골형성 단백질 2/4/7 성장인자에 대한 관찰을 통해 bio-glass의 골형성 기전을 알아보고 이를 자가골과 비교 평가하여 인공치아를 위한 골이식 재료로 bio-glass의 임상적 가능성을 알아보고자 한다. 방법 : 실험동물로는 동일조건하에서 사육된 체중 약 3.0kg(15주)내외의 New Zealand 가토 16마리를 두개 정중부에 원형 결손부를 두 곳에 형성하여 한쪽은 자가골 이식을 시행하고, 반대측 골 결손부는 Bio-glass(Biogran 3i, USA)단독으로 이식을 시행한 후 조직학적, 면역조직학적 관찰을 시행하였다. 결과 : 1. 신생골의 형성은 자가골과 Bio-glass 이식체에서 2주부터 관찰되었으나, Bio-glass에서는 일관된 증가양상을 보여주지 못했다. 2. 자가골 이식시 BMP 2/4/7의 발현은 4주에서 가장 많이 관찰되었다. 3. Bio-glass 이식시 BMP 2/4/7의 발현은 12주에서 가장 많이 관찰되었다. 결론 : Bio-glass 신생골 형성 시기는 자가골과 비슷하였으나, 시간 경과에 따라 신생골 형성율은 차이가 커졌으며 BMP발현은 자가골에 비해 늦어지거나 관찰되지 않았다. 그러므로, 골 이식시 Bio-glass의 선택은 신중해야 하며 생체 내에서의 작용에 대한 계속적인 임상 및 기초적인 연구가 필요할 것으로 사료된다.; Purpose : Bio-glass proved to be grafting substitute material, providing a reasonable bio-degradation time, no need for harvesting procedure. The purpose of this study is to evaluate application of Bio-glass as a bone graft material for grafting procedure. Material & Methods : Sixteen rabbits were divided into 4 groups of 4 period. The autogenous bone and bio-glass were grafted in Rabbit cranial defects. The rabbits were sacrificed at 2, 4, 8, and 12 weeks, and the histologic, immunohistochemical analysis were used to investigate bone regeneration phase. Results : 1. The new bone formation around autogenous bone and bio-glass was seen from 2 weeks, but bio-glass part was not seen incremented for each period. 2. In autogenous bone graft, 2/4/7 expression was most increased at 4 weeks. 3. In bio-glass graft, BMP 2/4/7 expression was most increased at 12 weeks. Conclusion : We considered that bio-glass can`t be a suitable material for grafting & augmentation at defect area, although the more research is needed.
