Abstract
본 연구는 아토피피부염 환아 양육에 중대한 역할을 하는 어머니를 대상으로 어머니의 사회적 지지, 불안과 우울의 정도를 파악하고 사회적 지지, 불안과 우울의 관련요인을 살펴보아, 아토피피부염 환아 어머니에 대한 심리적 지지를 위한 기초자료를 제공하고, 아토피피부염 환아 간호중재에 보다 실증적인 자료를 제공하기위해 시도된 서술적 상관관계 연구이다. 연구의 도구는 박지원(1985)의 사회적 지지 측정도구, Rajka & Langeland (1989)의 아토피피부염의 임상적 분류법, 육성필, 김중술(1997)에 의해 한국어로 표준화된 BAI, 이영호와 송종용(1991)에 의해 번안된 BDI가 사용되었으며, 자료수집은 아토피피부염으로 진단받고 서울시내에 소재한 S대학병원 소아피부과 아토피 크리닉을 방문한 0세-학령전기(만6세) 환아의 어머니중 115명을 대상으로 구조화된 설문지를 사용하여 2007년 3월 13일부터 4월 27일까지 실시하였다. 수집된 자료는 SPSS WIN 12.0을 이용하여 분석하였다. 연구결과를 요약하면 다음과 같다. 첫째, 환아 어머니의 사회적 지지정도는 평균 57.26점(±8.68)으로 중정도의 지지를 받고 있는 것으로 나타났고, 불안 정도는 11.90점(±9.46), 우울 정도는 11.21점(±8.75)으로 경한 정도의 불안과 우울을 보였다. 둘째, 사회적 지지, 불안과 우울간의 관계에서 환아 어머니의 사회적 지지와 불안(r=-.516, p= .000)은 중정도의 음의 상관관계를 보였으며, 환아 어머니의 사회적 지지와 우울(r=-.705, p= .000)은 높은 음의 상관관계를 보였고, 환아 어머니의 불안과 우울(r= .735, p= .000)은 높은 양의 상관관계를 보였다. 셋째, 어머니와 아동의 일반적 특성별 어머니의 사회적 지지, 불안과 우울 정도의 차이를 분석한 결과 사회적 지지는 부양육자(F=2.698, p= .049)에 따라 유의한 차이를 보였고 불안은 월수입(F=3.535, p= .032), 아동의 나이(r=-.225, p= .016), 형제수(F=7.392, p= .001), 형제순위(F=6.798, p= .000)에 따라, 우울은 형제 수(F=4.02, p= .021)에 따라 유의한 차이를 보였다. 넷째, 아토피피부염의 임상적 양상에 따른 환아 어머니의 사회적 지지, 불안과 우울 정도의 차이를 분석한 결과 사회적 지지는 알레르기 질환의 가족력(t=-2.68, p= .008), 아토피피부염의 임상적 중증도(r=-.245, p= .008)에 따라 차이를 보였고 불안은 유병기간(F=3.728, p= .013), 어머니가 느끼는 환아의 소양감(r= .326, p= .000)과 수면장애(r= .339, p= .000), 임상적 중증도(r= .406, p= .000)에 따라, 우울은 유병기간(F=2.925, p= .037), 알레르기 질환의 가족력(t=2.19, p= .031), 어머니가 느끼는 환아의 소양감(r= .225, p= .016)과 수면장애(r= .304, p= .001), 임상적 중증도(r= .479, p= .000)에 따라 유의한 차이를 보였다. 결론적으로 아토피피부염 환아 어머니의 사회적 지지, 불안과 우울 정도와 사회적 지지, 불안과 우울에 영향을 미치는 어머니와 아동의 일반적 특성과 임상적 양상을 파악함으로써 이를 고려한 환아 어머니에 대한 정서적인 지지가 이루어져야 하며 환아 어머니의 사회적 지지와 불안, 우울이 서로 밀접한 상관관계를 나타내므로 어머니의 불안과 우울을 감소시키는 중재변인으로 사회적 지지를 활용한 간호를 제공해야 한다는 것을 알 수 있었다. 연구 결과를 토대로 다음과 같은 제언을 하고자 한다. 아토피피부염 환아 어머니의 문제점을 더욱 깊이 파악하고 증진시킬 수 있도록 반복 연구와 심층적인 연구가 뒤따라야 하며, 여성의 사회진출 및 핵가족화를 고려하여 부모 혹은 중요한 양육 대행자를 포함한 가족단위의 확대 연구가 필요하다. 또한 장기적으로 사회적 지지, 불안과 우울의 변화되는 과정을 탐색, 분석할 수 있는 종단적 연구가 이루어져야 할 것이다.; This research provides basic data for mental support to mothers of children with atopic dermatitis by understanding the mother's social support and level of anxiety and depression and looking at the related factors on anxiety and depression. In addition, this research is a descriptive interrelationship research attempted to provide a more empirical data on mediating nursing of children with atopic dermatitis. The Social support Inventory which was made by Park, J. W.(1985), BDI adapted by Lee, Y. H. and Song, J. Y.(1991), BAI standardized in Korean by Yook, S. P. and Kim, J. S. (1997) and Rajka & Langeland (1989)'s clinical classification method were used as measurement tools of this research. Data was collected by a structured survey carried out on 115 mothers of children with atopic dermatitis of the ages 0~6 (before school period) who were diagnosed as atopic dermatitis and visited children dermatology department atopy clinic of S University Hospital between 13. March. 2007 to 27. April. 2007. The collected data was analyzed using SPSS WIN 12.0. The research result can be summarized as follows. First, the social support level in mothers of children with atopic dermatitis turned out to be in average 57.26 points (±8.68) and anxiety level and depression level turned out to be 11.90 points (±9.46) and 11.21 points (±8.75) respectively. Secondly, on the relationships between social support and anxiety and depression, social support in mothers of children with atopic dermatitis and anxiety(r=-.516, p= .000) showed a middle level of negative interrelationship, social support in mothers of children with atopic dermatitis and depression(r=-.705, p= .000) showed a high level of negative interrelationship and anxiety in mothers of children with atopic dermatitis and depression(r= .735, p= .000) showed a high positive interrelationship. Thirdly, after analyzing the degree of social support, anxiety and depression according to general characteristics of mothers and children with atopic dermatitis, it was found that social support shows a statistically significant difference by sub-rearer(F=2.698, p= .049), anxiety shows a statistically significant difference by monthly income(F=3.535, p= .032), child's age(r=-.225, p= .016), and number(F=7.392, p= .001) and order(F=6.798, p= .000) of brothers and sisters, depression shows a statistically significant difference by number of brothers and sisters(F=4.02, p= .021). Fourthly, as a result of analyzing the degree of social support, anxiety and depression according to clinical features of atopic dermatitis, it turned out that social support showed a statistically significant difference depending on the family history of allergy disease(t=-2.68, p= .008) and Severity of atopic dermatitis(r=-.245, p= .008), anxiety showed a statistically significant difference according to period of the disease(F=3.728, p= .013), children's pruritus(r= .326, p= .000) and loss of sleep(r= .339, p= .000) felt by mothers of children with atopic dermatitis , Severity of atopic dermatitis(r= .406, p= .000) and depression showed a statistically significant difference according to period of the disease(F=2.925, p= .037), family history of allergy disease(t=2.19, p= .031), children's pruritus(r= .225, p= .016) and loss of sleep(r= .304, p= .001) felt by mothers of children with atopic dermatitis, Severity of atopic dermatitis(r= .479, p= .000). In conclusion, mothers of children with atopic dermatitis need to be mentally supported by understanding to general characteristics of the mother and the child with atopic dermatitis affecting social support in mothers of children with atopic dermatitis, level of anxiety and depression and social support and anxiety and depression. In addition, the fact that is a close interrelationship between mothers of children with atopic dermatitis, anxiety and depression indicates that nursing using social support as the mediating factor reducing anxiety and depression of mothers need to be provided. Based on the research result, the following suggestions can be made. Repetition of this research and researches in depth need to be carried out in order to understand more fully about and improve the problems of mothers of children with atopic dermatitis and extended research on family units including parents and important rearing agents is required considering the change to a nuclear family. In addition, in the long term a vertical research which can observe and analyze the changing process of social support, anxiety and depression need to be implemented.
