Abstract
Because the korean population is aging fast, the burden of long-term care for old people is becoming an economical strain to old people and their families and causes a negative impact on national health insurance finance. Many advanced countries faced to these problems already, and introduced various type of long-term care system for older people. Specially, Germany and Japan are enforcing a long-term care system that is designed under social insurance method. The experiences of these countries can give a lot of suggestions to the development of the Korean long-term care insurance. From this perspective, Korean long-term care insurance can develop its social security system by comparing Germany, Japan and Korean longterm care insurance through analyzing introduction background, application target, contents of service, transmission system, financial resources supply applying policy analysis frame of Gilbert and Specht. The thesis finds the following: First, aging of population, limitation of medical treatment protection by family, limitation of existing system, Country(local government inclusion) finance and National health insurance finance aggravation are the common background. The difference between korea and other countries is that Korea introduced the new system in aging society-stage rather than the aged-society. Second, when it comes to targeted beneficiaries, Germany practice a system that is generally universal, whereas Japan and Korea are applying a selective system. Also, Germany and Japan apply the same burden responsibility and benefit right, while Korea does not. Third, the content of service is diverse in each country, and Germany is recognizing all goods and service, cash benefits, mixing service, And it is actively encouraging cash benefits and In-home services. Japan is implementing goods and service in principle. Cash benefits are recognized in only exceptional situations, and has service system that is encouraging In-home service than facility-based services. Also, Korea does goods and service in principle and recognize cash benefits exceptionally. Fourth, in transmission system, Germany is "Pflegekassen", and Korea is "National Health Insurance Corporation", and Japan is "Local Government", as their administration operation subject. Germany is training service workers in Federal Government standard, and Japan is training "Care manager" in country qualification system, However, Korea does not have any detailed regulations on human development and training. Fifth, in terms of financial resources supply, Germany is insurance premium 100%, Japan is consisted of each 50% of insurance premium and public finance(government and local-government subsidy), and Korea is consisted of insurance premium 80% and government subsidy 20%. Study finds that Korea's long-term care insurance development depends on the same burden responsibility and benefit right, service system construction that can be filled in various long term care demands, simplification of decision system and decision tool for estimation, quality elevation of service through long-term care professional manpower standardization and various other plans for stability of finance etc.; 한국은 인구의 고령화가 매우 빠르게 진행되고 있어 노인의 장기요양보호 부담은 노인과 그 가족에게 과중한 경제적 부담을 주고, 국민건강보험 재정에 악영향을 끼치고 있다. 많은 선진국들이 이미 이러한 문제에 직면하였었고, 문제해결을 위해 다양한 노인장기요양보호 제도를 도입하였다. 특히, 독일과 일본은 사회보험방식인 장기요양보험과 개호보험을 도입하여 시행하고 있다. 이러한 국가의 경험은 한국 노인장기요양보험의 발전방향에 많은 시사점을 줄 수 있다. 이러한 관점에서 본 연구는 독일, 일본, 한국의 노인요양보험제도를 Gilbert와 Specht의 정책분석틀을 적용하여 도입배경, 적용대상, 급여의 내용, 전달체계, 재원조달로 구분하여 비교·분석함으로써 한국의 노인장기요양보험이 실천적 사회보장제도로 발전할 수 있는 방향을 도출하고자 하였다. 본 논문의 연구결과는 다음과 같다. 첫째, 도입배경의 공통점으로 인구의 고령화, 가족에 의한 요양보호의 한계, 기존 제도의 한계, 국가(지방자치단체 포함) 재정 및 건강보험재정 악화 등으로 나타났다. 상이점으로는 독일과 일본은 고령사회에서 제도를 도입하였으나 한국은 고령화사회에서 도입하여 고령화 수준만을 놓고 볼 때 빠른 것으로 나타났다. 둘째, 적용대상에 있어서는 독일과 일본은 부담의무와 수급권이 일치하고 있는 반면, 한국은 부담의무와 수급권이 불일치하고 있다. 또한, 독일은 모든 국민에게 수급권을 부여하고 있지만 일본과 한국은 수급권을 특정연령 계층으로 한정하고 있으며 연령계층별로 요양의 범위가 다르게 구성되어 있다. 셋째, 급여의 내용은 각 국별로 상이하였으며, 형태에 있어서도 독일은 현물급여, 현금급여, 혼합급여를 모두 인정하고 있고 현금급여와 재가급여를 적극 권장하고 있다. 일본의 경우는 현물급여를 원칙으로 하고 현금급여는 극히 예외적인 경우에만 인정하고 있으며, 시설급여보다는 재가급여를 권장하는 급여체계를 가지고 있다. 또한 한국도 현물급여를 원칙으로 하고 예외적으로 현금급여를 인정하고 있다. 넷째, 전달체계에 있어서 관리운영주체는 독일의 경우 장기요양금고, 한국은 국민건강보험공단, 일본은 시·정·촌으로 되어 있다. 서비스제공 전문 인력을 독일은 연방차원의 표준을 설정하여 양성하고 있으며, 일본은 국가자격제도로 ‘개호복지사’를 양성하고 있으나, 한국은 아직까지 전문 인력에 대한 구체적인 규정이 마련되어 있지 않다. 다섯째, 재원조달의 방법에 있어서 독일은 보험료 100%, 일본은 보험료와 공비가 각각 50%로 구성되어 있고, 한국의 경우는 보험료 80%, 국고부담 20%로 구성되어 있다. 연구결과 한국 노인장기요양보험의 발전에 주는 시사점은 부담의무와 수급권의 일치, 다양한 장기요양 욕구에 부응한 급여체계 확보, 평가 판정절차 및 판정도구의 간소화, 장기요양 전문인력 표준화를 통한 서비스의 질 향상, 다양한 재정안정화 방안 도입 등으로 나타났다.
