Abstract
퇴행성 관절염 환자에게 범이론적 모델을 이용한 타이치 운동프로그램을 적용함으로서 그 효과를 검증하고, 퇴행성 관절염 환자에게 효율적인 운동중재 프로그램을 모색하기 위해 시도된 비동등성 대조군 전후 시차설계의 유사실험 연구이다.; This study tries to verify that there is an effect by applying TAI CHI exercise program using Transtheoretical Model(TTM) to degressive arthritis patients and to seek an efficient exercise intervention to degressive arthritis patients. The research design utilized for this study was a nonequivalent control group pretest-posttest design. A data collection was composed through two welfare institutions's aid for elders in C-city for 16 weeks between 1st of June and 22nd of September. Participants who have suffered degressive arthritis has not taken regular exercise. There is a total of 47 participants, 25 people in experimental group and 22 people in control group. The TAI CHI exercise program using TTM, which consists of phone counselling by stage-based exercise behavior protocol and also Tai Chi exercise with a duration of 16 weeks. The Tai Chi exercise was conducted to every weeks with a duration of 60 minutes each session two times a week, after the exercise behavior changes, participants shall apply for a phone counselling by the stage-based of exercise behavior protocoal to the experimental group every week for 10 to 15 minutes, and the control group was educated about arthritis for 3 weeks The effects of TAI CHI exercise program using TTM were measured by pain, physical states (shoulder and back flexibility, grip, back, leg strength and balance), self efficacy, life satisfaction, activity of daily living and stages of exercise behaviors. The collected data was analyzed using descriptive statistics, Χ²-test, t-test, repeated measures ANOVA by SPSS/WIN 13.0 program. 1. The effects of TAI CHI exercise program using TTM are listed below: Hypothesis 1. "The experimental group who participate in the TAI CHI exercise program using TTM would have less pain than the control group." was supported(F=13.796, p=.001). Hypothesis 2. "The experimental group who participate in the TAI CHI exercise program using TTM would improve physical states than the control group." Hypothesis 2-1. "The experimental group who participated TAI CHI exercise program using TTM would increase grip strength than the control group." was supported(F=21.473,p=.000). Hypothesis 2-2. "The experimental group who participated in the TAI CHI exercise program using TTM would increase back strength than the control group." was supported (F=13.207, p=.001). Hypothesis 2-3. "The experimental group who participated in the TAI CHI exercise program using TTM would increase leg strength than the control group." was supported (F=11.241, p=.002). Hypothesis 2-4. "The experimental group who participated in the TAI CHI exercise program using TTM would increase shoulder flexibility than the control group." was supported(F=12.135, p=.001, F=5.775, p=.020). Hypothesis 2-5. "The experimental group who participated in the TAI CHI exercise program using TTM would increase back flexibility than the control group." was rejected(F=0.476, p=.494). Hypothesis 2-6. "The experimental group who participated in the TAI CHI exercise program using TTM would increase balance than the control group." was supported(F=69.747, p=.000). Hypothesis 3. "The experimental group who participated in the TAI CHI exercise program using TTM would increase self efficacy than the control group." was supported(F=6.953, p=.011). Hypothesis 4. "The experimental group who participated in the TAI CHI exercise program using TTM would increase life satisfaction than the control group." was supported(F=25.732, p=.000). Hypothesis 5. "The experimental group who participated in the TAI CHI exercise program using TTM would improve activity of daily living than the control group." was supported(F=41.307, p=.000). 2. The degressive arthritis patients who has participated in the TAI CHI exercise program using TTM has shown there shall be moved to the action & maintenance on the stages of exercise behavior. In this research, the experimental group who participated in the TAI CHI exercise program using TTM has improved significantly in physical states (shoulder flexibility, grip, back, leg strength and balance), pain, self efficacy, life satisfactions and activity of daily living than the control group except the back flaxibility, and has shown changes in the moving stages of the exercise behaviors. Therefore TAI CHI exercising programs using TTM will be expected to be used in clinical practices as an intervention for health promotion that helps along not only motivation of exercise behavior but also exercise participation to degressive arthritis patients.
