임종환아를 돌보는 소아중환자실 간호사의 경험

Abstract

소아중환자실 간호사는 생명위기에 처한 중증도가 높은 환아들을 간호하고 있기 때문에 많은 죽음을 경험하고 간호사의 역할에 갈등을 느낀다. 간호사는 임종과정에서 자신과의 갈등을 해결할 수 있는 능력 즉, 삶과 죽음에 관한 긍정적인 가치관을 확립하고 있어야 전문적인 간호사의 역할을 수행할 수 있다. 본 연구의 목적은 임종환아를 돌보는 소아중환자실 간호사의 경험을 이해하고 그들이 바라는 방향을 확인하여 임종간호 수행에 있어 긍정적인 가치관 확립과 해결방안을 마련하기 위한 기초자료의 제공을 목적으로 수행되었다. 본 연구의 문제는 “임종환아를 돌보는 소아중환자실 간호사의 경험은 무엇인가?” 이다. 자료 수집 기간은 2007년 11월 1일에서 2008년 3월 10일까지였으며, 본 연구의 참여자는 소아중환자실에서 3년 이상 근무하면서 임종환아에 대한 간호경험이 풍부하고 본 연구의 목적을 잘 이해하여 능동적으로 임종환아 간호경험을 이야기 해 줄 수 있다고 생각되는 임상간호사 9명으로 구성되어 있다. 자료 수집의 방법은 참여자와의 개별적인 심층면담이었으며 수집된 자료는 Colaizzi(1978)제시한 방법을 사용하여 분석되었는데, 자료 분석의 결과 9가지의 주제모음과 4가지의 범주가 나타났으며 각각의 범주는 임종간호에 대한 부담감, 관계에서 오는 어려움, 다양한 정서경험, 바람직한 변화의 기대로 구성되었다. ‘임종간호에 대한 부담감’ 범주는 <죽음에 대한 부담감>, <사후처치에 대한 어려움>, <무의미한 치료에 대한 갈등>의 3가지 주제 모음을 포함한다. ‘관계에서 오는 어려움’ 범주는 <환아와의 관계>, <보호자와의 관계>의 2가지 주제 모음을 포함한다. ‘다양한 정서 경험’ 범주는 <임종간호에 대한 정서적 느낌과 반응>의 주제 모음을 포함한다. ‘바람직한 변화의 기대’ 범주는 <소아중환자실 간호사의 태도 개선>, <전문적인 임종간호교육의 필요성을 절감>, <지원체계 구축안 제시>의 3가지 주제모음을 포함한다. 이상의 결과에서 나타난 임종간호를 수행하면서 겪는 소아중환자실 간호사의 부담감과 어려움 등을 이해하고 그들이 바라는 방향을 확인하여 바람직한 해결방안을 마련한다면 간호사와 환자, 보호자와의 관계 형성과 의료인으로서의 긍정적인 가치관을 확립하는데 도움이 될 것이라 사료된다. 앞으로 임종환자를 간호하는 간호사들의 의견을 수렴할 수 있는 지지체계와 전문적인 간호사로서의 역할 수행에 도움이 되는 다양한 간호교육을 제공하고 스트레스 완화를 위한 프로그램등도 개발되어야 할 것이다.; Nurses working in pediatric intensive care unit, have conflicts as they nurse children patients who have serious disease enough to lose life, and they experience many of them losing life in fact. Nurses in pediatric intensive care unit must have ability for mind control of self-conflict from observing the procedure of deathbed children, so that they can perform their duty as professional deathbed nurse. The purpose of this study is to understand the experiences of nurses in pediatric intensive care unit who nurse children in deathbed and find out what they want and need in performing deathbed nurse and offer basic information to suggest solution and establishment of affirmative mind. The question of this study is "What do nurses experience in pediatric intensive care unit as nursing deathbed children?" The period of collection data was from November 1, 2007 to March 10, 2008. And participants in this study were 9 clinical nurses who worked more than 3 years in pediatric intensive care unit and had abundant experiences in nursing deathbed children and understood the purpose of this study well. The method of collecting data was deep personal interview, and the collected data were analyzed by method Colaizzi(1978) introduced. As the result of the data analysis, 9 subjects group and 4 categories were showed up. And the categories were burden of deathbed nursing, difficulties from relationship, experiences of various emotion, and expectation of desirable change. 'burden of deathbed nursing' category consisted of 3 subjects group like <burden of death>, <feeling difficulty in management after death>, and <conflict from meaningless treatment>. 'Difficulties from relationship' category consisted of 2 subjects group like <relation with child patient>, and <relation with parents>. 'Experiences of various emotion' category consisted of 1 subject like <emotional feeling and respond in nursing>. 'Expectation of desirable change' category consisted of 3 subjects group like<necessity of professional training of deathbed nurse>, <suggestion of supporting system>, and <Improvement of nurses' attitude in child intensive care unit>. If the above burden and difficulties of deathbed nursing is understood and proper solution is prepared after ascertaining what they want and need. it will be great help to set up relation between nurses and patients/ parents and affirmative minds as medical professionals. In the future, supporting system that can collect nurses' opinion and various professional training course that is necessary for nurses' advanced performance and program for stress relief should be developed.
