치매노인 부양자가 지각하는 사회적 지지와 부담감

Abstract

본 연구는 치매노인 부양자가 지각하는 사회적 지지와 부담감을 파악하고 이 두 변인간의 상관관계를 파악하여 부양감을 경감시키는 간호중재방안을 마련함에 있어 근거자료를 제공하고자 시도되었다. 자료수집기간은 2008년 9월 15일부터 10월 15일까지이며, 서울시 K구에 거주하는 치매주간보호센터 이용자와 치매주간보호센터 입소를 희망하는 치매노인의 가족부양자 82명을 대상으로 하였다. 연구 도구는 Sherbourn과 Stewart(1991)가 개발한 MOS(Medical Outcomes Study)를 마범순(1993)이 수정한 사회적 지지 척도 도구와 Novak과 Guest(1989)의 다차원적 부양자 부담 사정도구(Caregiver Burden Inventory: CBI)와 Kosberg와 Cairl (1986)의 간호비용 지표(Cost of Care Index: CCI)를 사용하였다. 수집된 자료는 SPSS WIN 14.0K 프로그램을 이용하였으며, 기술통계분석, Pearson’s Correlation, t-test와 ANOVA를 통해 분석하였다. 연구의 결과는 다음과 같다. 1) 치매노인 부양자의 사회적 지지와 부양 부담감은 5점 중 각각 2.65(±.92)와 2.86(±.70)이었으며, 사회적 지지의 하부 영역에서는 가족의 지지(3.11±1.24)가 가족 이외의 지지(2.18±1.78)보다 높은 것으로, 부양 부담감의 하부 영역에서는 시간의존성(3.73±.63)이 가장 높고, 사회적 부담감(2.36±.93)이 가장 낮은 것으로 나타났다. 2) 지각된 사회적 지지가 높을수록 부담감이 유의하게 낮은 것으로 나타났으며(r=-.434, p=.000), 신체적 부담감을 제외한 모든 하부 영역에서 유의한 역 상관관계를 보였다. 3) 부양자의 특성에 따른 사회적 지지의 차이에서는 연령, 월수입에 대한 생각, 주간보호센터 이용여부, 주간보호센터 이용시간이 통계적으로 유의한 차이를 나타났다. 4) 대상자의 특성에 따른 부담감의 차이에서는 성별, 종교, 교육정도, 주관적 건강, 월수입에 대한 생각에서 유의한 차이를 보였다. 이러한 결과를 종합하면, 부양자의 사회적 지지 중 가족의 지지가 중요하며, 치매노인을 돌보는데 부양자의 시간이 많이 소요될 때 부담감이 가장 큰 것으로 나타났다. 또한 사회적 지지가 높을수록 부양부담감이 낮으므로 부담감을 낮추기 위해서는 사회적 지지 중 가족의 지지를 높일 수 있는 전략 개발이 필요할 것으로 생각된다.; The purpose of this study is to describe social support and burden of caregivers of the elderly with dementia and to analyze relationship between perceived burden and social supports. The data were collected from 15th of September to 15th of October 2008. The convenient sampling was done from the 82 spouses who take care for demented people at the day care center in K-gu in Seoul. I used adjusted MOS(Medical Outcomes Study) tool developed by Sherbourn and Stewart and adjusted later by Ma, Bum Soon, CBI(Caregiver Burden Inventory) of Novak and Guest and CCI(Cost of Care Index) of Kosberg and Cairl. Collected data were analyzed using SPSS WIN 14.0K. The general characteristics of the subjects were analyzed by frequency, percentage, mean and standard deviation. Correlation with variable was analyzed using Person’s correlation coefficient, and social supports and burden of caregivers according to general characteristics of demented people and caregivers were analyzed using t-test and ANOVA. The results of this study are as follow. 1) Social support and burden of caregivers of demented people were 2.65(±.92) and 2.86(±.70) each. In social support, support from family(3.11±1.24) was higher than supports from people other than family(2.18±1.78), and in burden of caregivers, time dependency(3.73±.63) was highest and social burden(2.36±.93) was lowest. 2) There was statistically significant negative correlation(r=-.434, p=.000) between social support and burden, except physical burden. 3) In the relationship between social support according to whole characteristics of subjects, there was a statistically significant difference in age, opinion about monthly income, use of day care center, and hours in day care center. 4) In the relationship between burden of caregivers according to whole characteristics of subjects, there was a statistically significant difference in gender, religion, educational level, personally perceived health, and opinion about monthly income. In conclusion, this study shows that there is a relation between social support and burden of caregivers of older adults with dementia. This suggests that social support alleviates burden of caregivers in many ways. For that reason, there comes a necessity to efficient nursing intervention which relieve burden of caregivers and strengthen social support to caregivers.
