소아백내장 수술 후 굴절력의 변화

Abstract

목적 소아 백내장 수술 후 굴절력의 변화에 대해 알아보고자 하였다. 대상과 방법 1996년 1월부터 2003년 12월까지 한양대학교병원에서 소아 백내장으로 수술 후 5년 이상 추적관찰된 26명, 34안을 대상으로 의무기록을 후향적으로 분석하였으며 수술 후 1달에서 5년 사이에 굴절력의 변화를 알아 보았다. 결과 각 군의 굴절력의 변화는 2-3세, 4-5세, 6-7세, 8세 이상에서 각각 -1.23±1.09 D, -1.13±1.57 D, -0.27±0.65 D, -0.23±0.65 D 이었다. 수술시 연령에 따른 굴절력 변화와 각 연령군에서 단안과 양안의 굴절력의 변화는 모두 통계학적으로 유의한 차이를 보이지 않았으며 술 전 검사 시 정시에 해당하는 인공수정체도수에 따른 굴절력의 변화와 수술 시 삽입한 인공수정체도수의 목표굴절치에 따른 굴절력의 변화도 통계학적으로 유의한 차이를 보이지 않았다. 결론 소아 백내장 수술 후 굴절력은 대체로 근시성 변화를 보이고 수술 연령이 어릴수록 굴절력의 변화 정도가 큰 경향이 있어 수술 시 연령에 따른 근시성 변화를 예상하여 인공수정체 도수를 결정해야 할 것으로 생각된다.; Purpose To investigate the refractive change after pediatric cataract operation Methods A retrospective analysis of the medical records of 26 patients, 34 eyes who underwent pediatric cataract operation from January 1996 to June 2003 at Hanyang University hospital was performed. We evaluated the refractive change from postoperative 1 month to 5 years. Results The refractive change of each age-matched group 2-3, 4-5, 6-7, 8 years old was -1.23±1.09 D, -1.13±1.57 D, -0.27±0.65 D, -0.23±0.65 D respectively. In the age-matched subset of patients, no statistically significant difference in the refractive change were found between unilateral and bilateral cataracts. Conclusions The refractive change after pediatric cataract operation tends to myopia and the younger the age of operation was, the greater the extent of refractive change was. In this regard, the ideal IOL-power is to be decided in the consideration of possible myopic changes according to the patient age at the time of surgery.
