망막전막 제거술 후 빛간섭단층촬영 및 미세시야계를 통한 구조와 시기능 간의 연관성 연구

Abstract

목적: 망막전막 환자에 대해 유리체 절제술 및 망막전막과 내경계막 제거술을 시행하고 수술 전후 시력 및 망막 두께, 망막 민감도의 연관성을 알아보고자 하였다. 대상과 방법: 망막전막에 대해 수술 받은 환자 중, 수술 전과 수술 후 2주, 1달, 2달에 시력을 측정하고 SLO (Scanning laser Microscope) 미세시야계 검사 및 스펙트럼 도메인 빛간섭단층촬영(OCT)을 시행한 19명 20안을 대상으로 하였다. 중심오목을 중심으로 하여 5x5의 25개 점을 자극하였으며, 망막지형도 상에서 이와 일치되는 25개 점의 망막두께를 측정하였다. 이 중, 중심오목에서 9개를 평균하여 시력과의 연관성을 알아보았다. 또한 수술 중 내경계막 제거 후 망막과 내경계막의 유착으로 하얗게 변한 경우를 WHITE가 있다고 하여 WHITE 유무에 따른 시력, 망막 기능, 두께 등을 비교해 보았다. 결과: 수술 후 시력, 망막민감도 및 망막두께는 통계적으로 유의한 연관성을 보이지 않았다. WHITE 유무에 따른 차이는 수술 전 망막두께가 WHITE가 있는 군에서 증가되어 있는 것 외에 의미 있는 결과를 보이지 않았다. 그러나 통계적으로 유의하지는 않았지만 시간의 흐름에 따른 시력과 망막민감도는 호전 및 악화의 면에서 비슷한 경과를 보였다. 결론: 망막전막 환자에서 망막전막 및 내경계막 제거 수술 전후에 시행할 수 있는 시기능 검사로 SLO 미세시야계를 고려해 볼 수 있을 것으로 생각된다. |Purpose: To investigate the role of structural and functional measurements(retinal thickness, retinal sensitivity, visual acuity) in the assessment of internal limiting membrane (ILM) peeling for the treatment of eyes with epiretinal membrane(ERM). Methods: Twenty eyes of nineteen patients with ERM who underwent pars plana vitrectomy with ILM peeling were studied prospectively. Visual acuity measurement, spectral-domain optical coherence tomography and Scanning laser microscope(SLO) microperimetry were performed before and 2 weeks and 2 months after surgery. And in case of we defined a whitish change after ILM peeling as WHITE sign to compare their results. Results: Although there was no significant correlations between visual acuity, retinal sensitivity and retinal thickness, there was similar courses between visual acuity and retinal thickness in their improvement and aggravation.
