노인의 자살사고 영향 요인 : 2015년 한국복지패널조사를 이용하여

Abstract

본 연구의 목적은 사회적 자본 이론과 생태학적 체계이론을 토대로 노인의 자살사고에 영향을 주는 요인을 규명하여 노인의 자살사고에 대한 이해를 돕고, 노인의 자살예방을 위한 간호중재 개발에 기초자료로 활용하고자 시행되었다. 본 연구는 2015년 10차 한국복지패널조사 자료를 이용한 이차분석연구이며 서술적 조사연구이다. 65세 이상 노인 4,814명의 자료를 활용하였으며 복합표본설계를 고려하여 서술적 통계, Rao-Scott 카이제곱 검정과 t 검정, 다변량 로지스틱 회귀분석을 시행하였다. 연구결과 대상자의 3.6%가 최근 1년 동안 자살에 대해 진지하게 생각해 보았고, 성별, 거주형태, 국민기초생활보장 수급 여부, 주관적 건강수준, 만성질환 유무, 장애 유무, 배우자 유무, 가족관계 만족도, 사회적 친분관계 만족도에서 자살사고 정도가 유의하게 차이가 나타났다. 노인의 자살사고에 영향을 미치는 요인은 우울, 국민기초생활보장 수급 여부, 가족관계 만족도로 나타났다. 그러므로 노인의 자살예방을 위해 우울의 조기발견과 적극적인 관리와 함께 가족관계 만족도를 증진시킬 수 있는 간호전략이 필요하다.The purpose of this study was to identify the predictors of suicidal ideation in the elderly on the basis of social capital theory and ecological model. This study was secondary analysis based on the data from the 2015 10th Korea welfare panel study (KOWEPS) with the subjects 4,814 elderly aged over 65. The data were analyzed using descriptive analysis, Rao-Scott chi-square test, t-test and logistic regression to take account of complex samples analysis with the SPSS/WIN 23.0 program. 3.6% of subjects had the experiences of suicidal ideation. There were significant differences in gender, living arrangements, recipient of basic livelihood security, perceived health status, chronic disease, disability, depression, presence of spouse, satisfaction in family relationship, satisfaction in social relationship between elderly who thought of suicide and elderly who did not think. It was determined that influencing factors included depression, recipient of basic livelihood security, satisfaction in family relationship. Based on these results, nursing strategies focusing on managing the depression and enhancing the satisfaction in family relationships are needed to prevent suicide for the elderly.
