dc.description.abstract아토피 피부염과 건선은 피부 장벽 기능장애, 각질형성세포의 증식 그리고 활성화된 면역 세포의 침윤을 특징으로 가지는 T 세포 매개의 만성 염증성 피부질환이다. 각질형성세포로부터 IL-1β, IL-25, IL-33, TSLP, 침윤된 T 세포로부터 IFNγ, IL-4, IL-13, IL-17, IL-22, IL-31과 같은 싸이토카인이 분비되어 면역세포의 침윤, 표피의 비대증식과 기능장애를 유발하여 질병의 증상을 나타낸다. 현재까지 보고된 치료적 접근은 피부조직을 통과할 수 있는 국소 전달이 가능한 Tacrolimus와 같은 저분자 의약품과 경구 투여가 가능한 전신 면역조절제인 Cyclosporin A, 코르티코스테로이드 가 있으나, 중등도 이상의 환자에게서는 효과가 낮고 장기 복용시 혹은 중단 시 부작용이 많다고 알려져 있다. 최근 FDA 승인을 얻은 바이오 의약품인 ustekinumab과 dupilumab은 싸이토카인 혹은 그 수용체를 표적으로 하며, 특히 두가지 싸이토카인을 동시에 차단할 수 있는 치료제로서 새롭게 각광받고 있다. 그러나, 이러한 항체 기반 신약은 투여 경로가 정맥주사 혹은 피하주사로 제한적이고, T 세포의 기능을 직접적으로 조절하지 못하는 한계가 있다. 따라서, 본 연구에서는 다양한 싸이토카인 신호 전달과 T 세포의 기능을 동시에 조절하면서 국소적으로 피부 전달이 가능한 치료 단백질을 개발하는 것을 전략으로 하였다. 이러한 연구를 위해, 첫째로, in silico 스크리닝을 통해 인간 단백질 유래의 새로운 경피전달 펩타이드 AP를 도출하였고, AP-융합 형광단백질은 각질형성세포, 섬유아세포 및 면역 세포에 높은 세포 내 전달 효율을 나타냈다. 또한, 피부를 통해 전달한 AP-dTomato 단백질이 쥐 및 인간 피부 모두에서 진피 및 표피로 전달됨을 확인하였다. 더 나아가, AP는 공유결합으로 연결된 단백질 뿐만 아니라 비공유결합으로 섞인 단백질 또한 생쥐의 피부세포와 조직을 투과해 단백질을 전달할 수 있었다. 둘째로, 이러한 피부전달 펩타이드를 활용하여, 세포내 싸이토카인 신호조절을 위해 다양한 STAT 의 탈인산화효소 단백질인TC-PTP 단백질을 재조합 단백질 (AP-rPTP) 로 제작하여, JAK1, JAK3, STAT1, STAT3, STAT5, STAT6와 같은 표적의 싸이토카인 신호전달 조절을 목적으로 연구를 진행하였다. AP-rPTP는 싸이토카인 자극 시 표피세포에서 IL-6, IL-4에 의한 pSTAT3, pSTAT6을, 비장 세포에서 각 IFNγ, IL-6, IL-2, IL-4에 의한 pSTAT1, pSTAT3, pSTAT5 및 pSTAT6의 인산화를 억제하였을 뿐만 아니라, T 세포의 활성화, 증식 및 싸이토카인 분비를 조절할 수 있음을 확인하였다. AP-rPTP는Th2, Th17 분화를 특이적으로 조절할 수 있음을 검증하였다. 이렇게, AP-rPTP의 다양한 싸이토카인 신호 조절 및 T 세포 기능 조절 능력을 바탕으로, AP-rPTP를 패치 방법으로 피부를 통해 전달한 결과 아토피 피부염 및 건선 유사 피부염 모델에서 피부 조직 비대증식, 염증 및 싸이토카인 발현을 상당히 억제시킬 수 있음을 확인하였다. 이 단백질의 예비 독성 시험에서 AP-rPTP 단백질에 의한 국소 독성 반응이 나타나지 않음을 규명하여 안전한 후보 약물이 될 수 있음을 알 수 있었다. 결론적으로, 본 연구는 9개 아미노산 길이의 새로운 경피전달 펩타이드인 AP를 도출하여 인간 피부에까지 경피전달이 가능한 펩타이드 기반 피부약물전달의 기술적 타당성을 입증하였다. 특히, 다양한 싸이토카인의 신호 조절과 T세포의 기능 조절을 동시에 할 수 있는 재조합 단백질 AP-rPTP를 제작하여 만성 염증성 피부질환인 아토피 피부염과 건선의 치료 효능을 검증하였다. 본 연구를 통해 경피전달이 가능한 바이오의약품 개발 전략 수립 및 난치성 만성 피부질환에 대한 신약 개발로 이어질 수 있을 것으로 기대한다.; Atopic dermatitis and psoriasis are the T-cell mediated chronic inflammatory skin diseases characterized by fundamental barrier dysfunction, keratinocyte hyperproliferation, and activated immune cell infiltration. Various cytokines like IL-1β, IL-25, IL-33, TSLP from keratinocytes and IFNγ, IL-4, IL-13, IL-17, IL-22, IL-31 from infiltrated T cells are secreted and induce symptoms of the disease caused by immune cell infiltration, epidermal hyperplasia and dysfunction. The therapeutic approach reported to date including small molecule drugs such as Tacrolimus, which can be delivered locally through skin tissue, and cyclosporin A, a systemic immunomodulator that can be administered orally, are not effective for moderate-to-severe patients and it is known that there are many adverse effects at long-term treatment or after discontinuation. Recent FDA-approved biologics, ustekinumab and dupilumab, are spotlighted as a therapeutics for blocking dual target by targeting cytokines or their receptors. However, these antibody-based new drugs have limited administration routes such as intravenous or subcutaneous injection and ability to directly control T cell functions. Therefore, in this study, transcutaneous delivery of macromolecules with controlling of JAK-STAT signaling and T cell function were adopted as a strategy to inhibit multiple signal transduction of cytokines and T cell function while allowing penetration of skin tissue. Firstly, this study identified a novel transdermal delivery peptide AP of human origin by in silico screening. AP-conjugated reporter proteins exhibited significant intracellular transduction efficacy in keratinocytes, fibroblasts, and immune cells. In addition, transcutaneous administration of AP-dTomato resulted in showed significant localization into the dermis and epidermis in both mouse and human skin. Furthermore, AP could delivery not only covalent-linked protein but also non-covalently-mixed proteins through the skin cells and tissue barrier. Secondly, by using this transdermal delivery peptide, the recombinant protein (AP-rPTP) was constructed with TC-PTP protein, which is a phosphatase protein targeting various STATs, and proceeded with this study for the purpose of regulating cytokine signaling like JAK1, JAK3, STAT1, STAT3, STAT5, and STAT6. AP-rPTP inhibited the phosphorylation of pSTAT3, pSTAT6 by IL-6, IL-4 stimulation in mouse epidermal cells and pSTAT1, pSTAT3, pSTAT5 and pSTAT6 in mouse splenocytes upon IFNγ, IL-6, IL-2, IL-4 cytokine stimulation, and also regulated T cell activation, proliferation and cytokine secretion. AP-rPTP was able to specifically regulate Th2 and Th17 differentiation. Based on the ability of AP-rPTP to regulate various cytokine signaling and T cell function, it was confirmed that epidermal hyperplasia, inflammation and cytokine expression were significantly suppressed by administration of AP-rPTP on the skin using a patch method in atopic dermatitis and psoriasis-like skin inflammation model. In the preliminary toxicity test of this protein, it was found that the local toxic reaction by AP-rPTP protein did not appear suggesting it could be a safe candidate drug. In conclusion, this study demonstrates the technical feasibility of peptide-based transdermal delivery that is applied to human skin by a novel 9-amino acids in length transdermal delivery peptide, AP. In particular, the recombinant protein AP-rPTP, which can simultaneously regulate cytokine signaling and T cell function, was constructed and elucidated its regulatory effect against inflammation in atopic dermatitis and psoriasis. This study is expected to lead to the establishment of a biologics development strategy capable of transdermal delivery and to the development of new drugs for intractable chronic skin diseases.-
