Abstract
양성자 빔을 사용하여 최적의 치료를 수행하고 환자의 안전을 제고하기 위해서는 치료 중 실시간으로 양성자 빔의 비정을 검증하는 것이 필요하다. 이를 위해 양성자 빔과 매질이 핵반응하여 발생하는 즉발감마선 분포를 측정하는 방법이 제안되었으며, 다양한 연구팀에서 즉발감마선 측정 장치를 개발하여 실험적으로 양성자 빔 비정 검증의 가능성을 확인하였다. 하지만 임상에서 활용할 만큼의 성능을 가진 최적화된 측정 장치가 개발되지 않았다. 본 논문에서는 임상 적용을 목표로 다중 슬릿 즉발감마선 카메라를 개발하였고, 실시간으로 빔의 비정을 검증할 수 있음을 실험적으로 확인하였다. 다중 슬릿 즉발감마선 카메라에서는 빔의 방향과 수직으로 발생하는 즉발감마선만을 선택적으로 검출하기 위해 144 mm 영상영역(field of view)을 갖는 다중 슬릿 집속장치를 제작하였고, 3 × 30 × 100 mm3 크기의 72채널 섬광검출기 배열을 제작하였다. 또한, 72채널 섬광검출기에서 측정한 즉발감마선 분포를 통해 빔의 비정을 결정하기 위하여 다채널 신호처리 시스템, 실시간 즉발감마선 분포 획득 프로그램, 빔 비정 결정 자동화 프로그램을 개발하였다. 개발한 카메라의 성능을 평가하기 위하여 국립암센터의 치료용 양성자 빔을 사용한 실험을 수행하였다. 먼저, 균질 팬텀에 95.09, 122.6, 146.45와 186.3 MeV의 양성자 빔을 조사하는 실험을 수행하였으며, 3.8 × 108개 이상의 양성자 빔을 조사하였을 때 불확실성은 일반적으로 1 mm 미만이며, EBT3 필름으로 측정한 실제 빔의 비정과의 오차가 3 mm 미만인 것을 확인할 수 있었다. 비균질 팬텀에서는 밀도가 높은 매질이 포함된 경우 빔의 비정을 2 mm 이내의 오차로 결정할 수 있었으나, 폐와 같이 밀도가 낮은 매질이 포함된 경우 결과가 좋지 않았으며 추가적인 연구가 필요하다. 마지막으로 RANDO 두경부 팬텀에 95.09, 108.71과 120.91 MeV 양성자 빔을 조사하여 빔의 비정을 결정하는 실험을 수행하였으며, 그 결과 2.5 mm 이내의 오차로 빔의 비정을 결정할 수 있었다. 본 연구의 결과는 150 MeV의 양성자 빔에서 사용하는 안전여백인 8.5 mm보다 더 정확하게 빔의 비정을 결정할 수 있었으며, 안전여백을 3.5% + 3 mm가 아닌 ± 3 mm로 줄여 최적화된 양성자 치료에 기여할 것이다. 본 연구를 통해, 다중 슬릿 즉발감마선 카메라가 치료용 양성자 빔에 대하여 실시간으로 빔의 비정을 검증할 수 있음을 실험적으로 확인하였고, 이를 바탕으로 카메라의 임상 적용 가능성을 확인하였다.; To fully utilize the advantage of proton therapy, it is required to verify the proton beam range in the patient in real time. One of the most promising methods for the proton beam range verification is to measure prompt gammas from the nuclear reactions of the proton beam in the patient. Until now, however, an optimized detection system for the clinical condition has not been developed. In the present dissertation, a multi-slit prompt-gamma camera was developed for clinical application and the developed camera was tested to see if the camera can verify the beam range in real time. To construct the multi-slit prompt-gamma camera for clinical application, we first constructed a multi-slit collimator with a 144 mm field of view and 72-channel CsI(Tl) scintillation detectors of 3 × 30 × 100 mm3. In addition, in order to process the signals from the 72-channel scintillation detectors and determine the beam range, we developed a multi-channel signal processing system, a real-time prompt-gamma distribution acquisition program, and an automated beam range determination algorithm. To evaluate the performance of the developed camera, a set of experiments using therapeutic proton beams from the proton therapy machine at the National Cancer Center in Korea were conducted. First, the proton beams of 95.09, 122.6, 146.45, and 186.3 MeV were delivered to a homogeneous phantom. When 3.8 × 108 or more protons were delivered, the uncertainty of the measured beam range was generally less than 1 mm, and the error from the actual beam range measured by the EBT3 film was less than 3 mm. Second, the proton beams of 95.09, 108.71, 112.52, 115.83, 125.56, 130.42, and 136.39 MeV were delivered to the heterogeneous phantom. In the heterogeneous phantom containing a layer of bone equivalent material, the error from the actual beam range was less than 2 mm after inhomogeneity correction. However, the results of the heterogeneous phantom containing a layer of lung equivalent material showed much larger errors, requiring further studies. Finally, the proton beams of 95.09, 108.71, and 120.91 MeV were delivered to a RANDO head phantom, and the error from the actual beam range was less than 2.5 mm. The results show that the camera can determine the beam range more accurately than the safety margin of 8.5 mm at 150 MeV and contribute to optimized proton therapy by reducing the safety margin to ± 3 mm instead of 3.5% + 3 mm. The results of this dissertation generally show that the developed multi-slit prompt-gamma camera can be used to verify the therapeutic proton beam range in the patient in real time, with some exceptions (i.e., lungs).
